How to do the End Screen for the Forms 501/502?

The End Screen is a mandatory part of the verification process for signed forms. You have two options for completing the End Screen:

• Printed
• Digital

Option 1: Printed

If you choose to complete the End Screen using a printed form, you must:

1. Print the PDF form that was sent to you by email.
2. Fill in the form by hand.
3. Place your ID document within the designed frame on the form.
4. Take a photo of:

• • Both completed pages of the form, and
  • The page with your ID visible in the frame.

Forms that are filled in digitally and then printed are not accepted.

Option 2: Digital

If you choose to complete the End Screen digitally, you must:

1. Open the form link provided in the email.
2. Fill in the form online.
3. Take a photo of the form displayed on your screen with your ID document held in front of it.

Digital forms are accepted only if signed through RightSignature.
