Form 501 is a mandatory document called the “Cryptocurrency Purchase Confirmation”, created to make sure you fully understand the details of your cryptocurrency purchase before finalizing it.

What You Need to Do

Option 1: Electronic Signature

Option 2: Printed Form

1. Print the blank Form 501 that was sent to you in PDF file.
2. Fill it in by hand (digitally filled forms that are then printed are not accepted).
3. Then, complete the End Screen by:
